except North Dakota, Ohio, Washington, West Virginia, and Wyoming.

Policy Premiums
Policy premiums are based on estimated annual payroll and the
nature of your business. “1099” wages should be
included unless the subcontractor has their own workers’
compensation insurance coverage.
